The aim of the present study was to investigate reactions in trapezius muscle blood flow (MBF), muscle activity, heart rate variability (HRV) and systemic blood pressure (BP) to autonomic tests in subjects with chronic neck-shoulder pain and healthy controls. Changes in muscle activity and blood flow due to stress and unfavourable muscle loads are known underlying factors of work-related muscle pain. Aberration of the autonomic nervous system (ANS) is considered a possible mechanism. In the present study, participants (n = 23 Pain, n = 22 Control) performed autonomic tests which included a resting condition, static hand grip test (HGT) at 30% of maximal voluntary contraction, a cold pressor test (CPT) and a deep breathing test (DBT). HRV was analysed in time and frequency domains. MBF and muscle activity were recorded from the upper trapezius muscles using photoplethysmography and electromyography (EMG). The pain group showed reduced low frequency-HRV (LF) and SDNN during rest, as well as a blunted BP response and increased LF-HRV during HGT (a dagger systolic 22 mm Hg; a dagger LF(nu) 27%) compared with controls (a dagger systolic 27; a dagger LF(nu) 6%). Locally, the pain group had attenuated trapezius MBF in response to HGT (Pain 122% Control 140%) with elevated trapezius EMG following HGT and during CPT. In conclusion, only HGT showed differences between groups in systemic BP and HRV and alterations in local trapezius MBF and EMG in the pain group. Findings support the hypothesis of ANS involvement at systemic and local levels in chronic neck-shoulder pain.

The aim of this study was to validate a non-invasive optical probe for simultaneous blood flow measurement at different vascular depths combining three photoplethysmography (PPG) channels and laser Doppler flowmeter (LDF). Wavelengths of the PPG were near-infrared 810 nm with source-to-detector separation of 10 and 25 mm, and green 560 nm with source-to-detector separation of 4 mm. The probe is intended for clinical studies of pressure ulcer aetiology. The probe was placed over the trapezius muscle, and depths from the skin to the trapezius muscle were measured using ultrasound and varied between 3.8 and 23 mm in the 11 subjects included. A provocation procedure inducing a local enhancement of blood flow in the trapezius muscle was used. Blood flows at rest and post-exercise were compared. It can be concluded that this probe is useful as a tool for discriminating between blood flows at different vascular tissue depths. The vascular depths reached for the different channels in this study were at least 23 mm for the near-infrared PPG channel (source-to-detector separation 25 mm), 10-15 mm for the near-infrared PPG channel (separation 10 mm), and shallower than 4 mm for both the green PPG channel (separation 4 mm) and LDF.

About 2% of the population suffer from heart failure, which is a disease associated with high mortality. We have developed a measurement platform including an interpretation tool for heart failure patients where physiological signals can be acquired and on which signal analysis techniques can be implemented. The platform can also be used to store patient data, to enable comparison over time and invoke distance consultation if necessary. In this platform, we have implemented a tool for interpretation support of the data measured from the patient. This tool are intended for use in home health care as an aid for monitoring and follow up heart failure patients.

Ultrasound Doppler using two-dimensional (2D) techniques is commonly used to study blood flow and myocardial tissue motion. This use includes measurement of velocity and time intervals, often in relation to the electrocardiogram (ECG) signal. 2D Doppler is frequently considered a real-time technique but in reality the acquisition time can be as long as 200 ms per image. We have developed a test-phantom using a rotating cylinder to simulate blood flow and tissue motion in a whole sector or space angle to evaluate velocity and timing characteristics.The phantom can produce constant velocities for velocity testing, as well as accelerating movement for testing the timing characteristics of ultrasound systems. Our investigation shows that the cylinder phantom is especially suitable for timing measurements in 2D Doppler imaging and that time delays between the Dopplersignals and the ECG signal exist in the tested ultrasound system.

Objective-To investigate use of signal analysis of heart sounds and murmurs in assessing severity of mitral valve regurgitation (mitral regurgitation [MR]) in dogs with myxomatous mitral valve disease (MMVD).

Animals-77 client-owned dogs.

Procedures-Cardiac sounds were recorded from dogs evaluated by use of auscultatory and echocardiographic classification systems. Signal analysis techniques were developed to extract 7 sound variables (first frequency peak, murmur energy ratio, murmur duration > 200 Hz, sample entropy and first minimum of the auto mutual information function of the murmurs, and energy ratios of the first heart sound [S1] and second heart sound [S2]).

Results-Significant associations were detected between severity of MR and all sound variables, except the energy ratio of S1. An increase in severity of MR resulted in greater contribution of higher frequencies, increased signal irregularity, and decreased energy ratio of S2. The optimal combination of variables for distinguishing dogs with high-intensity murmurs from other dogs was energy ratio of S2 and murmur duration > 200 Hz (sensitivity, 79%; specificity, 71%) by use of the auscultatory classification. By use of the echocardiographic classification, corresponding variables were auto mutual information, first frequency peak, and energy ratio of S2 (sensitivity, 88%; specificity, 82%).

Conclusions and Clinical Relevance-Most of the investigated sound variables were significantly associated with severity of MR, which indicated a powerful diagnostic potential for monitoring MMVD. Signal analysis techniques could be valuable for clinicians when performing risk assessment or determining whether special care and more extensive examinations are required.

An optical on-line monitoring system aimed at the estimation of dialysis dose has been tested clinically. The natural logarithmic slope is used to calculate Kt/V(urea) from ultraviolet (UV)-absorbance measurements. Errors in the calculation of Kt/V(urea) may appear due to changes in blood and dialysate flow or due to disturbances when the slope is used to estimate dialysis dose. This study introduces a new parameter for dialysis monitoring that may be used as a complementary parameter, the area under UV-absorbance curve (AUCa), to reflect a total solute removal during dialysis. The aim was to investigate the relationship between this new dialysis on-line monitoring parameter, AUCa, and the total removal of a few solutes. Fifteen patients were monitored during hemodialysis using UV absorbance at the wavelength of 297 nm. All spent dialysate passed through a flow cuvette in a spectrophotometer and then further to a collection tank where solute concentrations in the entire spent dialysate were determined. The AUCa at 297 nm was compared with the total amount of removed solute in the tank (reference method). The result shows strong correlations between AUCa and the total removal of urea, urate, creatinine, and phosphate during a given treatment and less strong correlation in all 15 patients together. A first indication of a new, possible, complementary parameter in hemodialysis treatment is presented, the AUCa, prospected to estimate solute removal.

We have previously developed a method for localization of the first heart sound (S1) using wavelet denoising and ECG-gated peak-picking. In this study, an additional enhancement step based on cross-correlation and ECG-gated ensemble averaging (EA) is presented. The main objective of the improved method was to localize S1 with very high temporal accuracy in (pseudo-) real time. The performance of S1 detection and localization, with and without EA enhancement, was evaluated on simulated as well as experimental data. The simulation study showed that EA enhancement reduced the localization error considerably and that S1 could be accurately localized at much lower signal-to-noise ratios. The experimental data were taken from ten healthy subjects at rest and during invoked hyper- and hypotension. For this material, the number of correct S1 detections increased from 91% to 98% when using EA enhancement. Improved performance was also demonstrated when EA enhancement was used for continuous tracking of blood pressure changes and for respiration monitoring via the electromechanical activation time. These are two typical applications where accurate localization of S1 is essential for the results.

In aortic valve stenosis (AS), heart murmurs arise as an effect of turbulent blood flow distal to the obstructed valves. With increasing AS severity, the flow becomes more unstable, and the ensuing murmur becomes more complex. We hypothesize that these hemodynamic flow changes can be quantified based on the complexity of the phonocardiographic (PCG) signal. In this study, sample entropy (SampEn) was investigated as a measure of complexity using a dog model. Twenty-seven boxer dogs with various degrees of AS were examined with Doppler echocardiography, and the peak aortic flow velocity (V_max) was used as a reference of AS severity. SampEn correlated to V_max with R = 0.70 using logarithmic regression. In a separate analysis, significant differences were found between physiologic murmurs and murmurs caused by AS (p < 0.05), and the area under a receiver operating characteristic curve was calculated to 0.96. Comparison with previously presented PCG measures for AS assessment showed improved performance when using SampEn, especially for differentiation between physiological murmurs and murmurs caused by mild AS. Studies in patients will be needed to properly assess the technique in humans.

Background: The non-invasive photoplethysmographic (PPG) signal reflects blood flow and volume in a tissue. The PPG signal shows variation synchronous with heartbeat (PPGc), as used in pulse oximetry, and variations synchronous with breathing (PPGr). PPGr has been used for non-invasive monitoring of respiration with promising results. Our aim was to investigate PPG signals recorded from different skin sites in order to find suitable locations for parallel monitoring of variations synchronous with heartbeat and breathing. Methods: PPG sensors were applied to the forearm, finger, forehead, wrist and shoulder on 48 awake healthy volunteers. From these sites, seven PPG signals were simultaneously recorded during normal spontaneous breathing over 10 min. Capnometry served as respiration and electrocardiogram (ECG) as pulse reference signals. PPG signals were compared with respect to power spectral content and squared coherence. Results: Forearm PPG measurement showed significantly higher power within the respiratory region of the power spectrum [median (quartile range) 42 (26)%], but significantly lower power within the cardiac region [9 (10)%] compared with the other skin sites. PPG finger measurement showed the opposite, in transmission mode, the power within the respiratory region was significantly lower [4 (10)%] and within the cardiac region significantly higher [45 (25)%] than the other sites. PPGc coherence values were generally high [>0.96 (0.08)], and PPGr coherence values lower [0.83 (0.35)-0.94 (0.17)]. Conclusion: Combined PPG respiration and pulse monitoring is possible, but there are significant differences between the respiratory and cardiac components of the PPG signal at different sites. © 2007 Acta Anaesthesiol Scand.

Clothing with conductive textiles for health care applications has in the last decade been of an upcoming research interest. An advantage with the technique is its suitability in distributed and home health care. The present study investigates the electrical properties of conductive yarns and textile electrodes in contact with human skin, thus representing a real ECG-registration situation. The yarn measurements showed a pure resistive characteristic proportional to the length. The electrodes made of pure stainless steel (electrode A) and 20% stainless steel/80% polyester (electrode B) showed acceptable stability of electrode potentials, the stability of A was better than that of B. The electrode made of silver plated copper (electrode C) was less stable. The electrode impedance was lower for electrodes A and B than that for electrode C. From an electrical properties point of view we recommend to use electrodes of type A to be used in intelligent textile medical applications. © International Federation for Medical and Biological Engineering 2007.

There is a growing interest in nonlinear analysis of respiratory sounds (RS), but little has been done to justify the use of nonlinear tools on such data. The aim of this paper is to investigate the stationarity, linearity and chaotic dynamics of recorded RS. Two independent data sets from 8 + 8 healthy subjects were recorded and investigated. The first set consisted of lung sounds (LS) recorded with an electronic stethoscope and the other of tracheal sounds (TS) recorded with a contact accelerometer. Recurrence plot analysis revealed that both LS and TS are quasistationary, with the parts corresponding to inspiratory and expiratory flow plateaus being stationary. Surrogate data tests could not provide statistically sufficient evidence regarding the nonlinearity of the data. The null hypothesis could not be rejected in 4 out of 32 LS cases and in 15 out of 32 TS cases. However, the Lyapunov spectra, the correlation dimension (D2) and the Kaplan-Yorke dimension (DKY) all indicate chaotic behavior. The Lyapunov analysis showed that the sum of the exponents was negative in all cases and that the largest exponent was found to be positive. The results are partly ambiguous, but provide some evidence of chaotic dynamics of RS, both concerning LS and TS. The results motivate continuous use of nonlinear tools for analysing RS data. © 2005 Elsevier Ltd. All rights reserved.

In this study, we investigate the beat-to-beat respiratory fluctuations in pulse wave transit time (PTT) and its subcomponents, the cardiac pre-ejection period (PEP) and the vessel transit time (VTT) in ten healthy subjects. The three transit times were found to fluctuate in pace with respiration. When applying a simple breath detecting algorithm, 88% of the breaths seen in a respiration air-flow reference could be detected correctly in PTT. Corresponding numbers for PEP and VTT were 76 and 81%, respectively. The performance during hypo- and hypertension was investigated by invoking blood pressure changes. In these situations, the error rates in breath detection were significantly higher. PTT can be derived from signals already present in most standard monitoring set-ups. The transit time technology thus has prospects to become an interesting alternative for respiration rate monitoring. © International Federation for Medical and Biological Engineering 2006.

Heart murmurs are often the first signs of pathological changes of the heart valves, and they are usually found during auscultation in the primary health care. Distinguishing a pathological murmur from a physiological murmur is however difficult, why an “intelligent stethoscope” with decision support abilities would be of great value. Phonocardiographic signals were acquired from 36 patients with aortic valve stenosis, mitral insufficiency or physiological murmurs, and the data were analyzed with the aim to find a suitable feature subset for automatic classification of heart murmurs. Techniques such as Shannon energy, wavelets, fractal dimensions and recurrence quantification analysis were used to extract 207 features. 157 of these features have not previously been used in heart murmur classification. A multi-domain subset consisting of 14, both old and new, features was derived using Pudil’s sequential floating forward selection (SFFS) method. This subset was compared with several single domain feature sets. Using neural network classification, the selected multi-domain subset gave the best results; 86% correct classifications compared to 68% for the first runner-up. In conclusion, the derived feature set was superior to the comparative sets, and seems rather robust to noisy data.

Background. An on-line monitoring system for dialysis dose calculations could make it possible to provide an adequate dialysis dose that is consistently given to haemodialysis (HD) patients. The aim of this study was to compare dialysis dose (Kt/V) using four different methods and their sensitiveness to a reduction in clearance.

Methods. Six patients were monitored on-line with ultraviolet (UV)-absorbance at a wavelength of 297 nm in three consecutive dialysis sessions during 1 week. During the last treatment, the clearance was reduced by 25% by decreasing the blood flow. For the determination of UV-absorbance, a spectrophotometer was connected to the fluid outlet of the dialysis machine with all spent dialysate passing through a flow cuvette. The equilibrated Kt/V (eKt/V) estimated by UV-absorbance was compared with eKt/V from the ionic dialysance method using the on-line clearance monitor (OCM) and the appurtenant software dose-calculation tool DCTool (Fresenius Medical Care, Germany), eKt/V calculated from the dialysate-urea slope and with eKt/V from pre- and post-dialysis blood-urea samples as reference.

Results. The study demonstrates that the sensitiveness to clearance reduction is similar in the four methods compared for eKt/V. When the different methods were compared, the mean eKt/V of UV-absorbance was 1.21 ± 0.20, blood 1.30 ± 0.21, dialysate 1.32 ± 0.21 and OCM (using the DCTool) 1.31 ± 0.21. The standard deviation was of the same magnitude.

Conclusion. The UV-method gives a similar response to clearance reduction compared with the other methods when comparing dialysis dose. The high sampling rate by continuous monitoring of UV-absorbance allows evaluation of the clearance process during dialysis and gives immediate feedback to on-line adjustments.

Objective: To evaluate a new technique using a photoplethysmographic (PPG) probe for automatic ankle pressure measurements. Design: Comparative study on two techniques for ankle pressure measurement. Setting University hospital. Material: Thirty-five patients with leg arterial disease and eight healthy volunteers. Ankle-brachial indices (ABPI) were measured using conventional CW Doppler technique and PPG-based prototype equipment for the ankle pressure recordings. Chief outcome measures: ABPIs calculated from CW Doppler and PPG ankle pressure measurements. The PPG signals were analysed both by visual judgement and by a software based, automatic algorithm. Main results: The mean difference between ABPIs calculated from CW Doppler recordings and PPG (visual analysis) was -0.01 (limits of agreement (±two standard deviations) +0.16 to -0.19). The correlation coefficient was 0.93. When the algorithm was used, the mean difference (CW Doppler-PPG) was 0.05 (limits of agreement 0.28 to -0.18, r=0.89). Conclusions: The PPG method is a promising technique with an inherent potential for automatisation of the ankle pressure measurements, thereby reducing the observer-dependency in ABPI recordings. © 2005 Elsevier Ltd. An automated method for ankle systolic pressure measurement, less operator dependent than the standard continuous wave (CW) Doppler technique, would imply an advantage both in patient measurements and in epidemiological studies. We present a new photoplethysmographic (PPG) probe that uses near-infrared light (880 nm) to detect pulsatory blood flow underneath the distal end of a standard pneumatic cuff. The probe is adapted to the anatomical conditions at the ankle, permitting recording of pressures in both ankle arteries separately. The validity of the equipment was tested with CW Doppler-derived systolic pressures and invasive blood pressure measurements for reference. In 20 healthy subjects, visual analysis of the PPG curves revealed a mean difference between CW Doppler and PPG measurements of -0.5 mmHg (SD 6.9). Corresponding results for the anterior and posterior tibial arteries separately were -1.8 mmHg (SD 6.2) and 0.9 mmHg (SD 7.3), respectively. A correct probe position was essential for the results. In direct recordings from the dorsalis pedis artery in 10 intensive care patients, PPG underestimated systolic pressure in the anterior tibial artery by 4.5 mmHg (SD 12.1). With further development, the PPG probe, integrated in the pneumatic cuff, may simplify measurements of ankle systolic pressures. © 2005 Biomedical Engineering Society.

Phosphorus-31 NMR spectroscopy using slice selection (DRESS) was used to investigate the absolute concentrations of metabolites in the human liver. Absolute concentrations provide more specific biochemical information compared to spectrum integral ratios. Nine patients with histopathologically proven diffuse liver disease and 12 healthy individuals were examined in a 1.5-T MR scanner (GE Signa LX Echospeed plus). The metabolite concentration quantification procedures included: (1) determination of optimal depth for the in vivo measurements, (2) mapping the detection coil characteristics, (3) calculation of selected slice and liver volume ratios using simple segmentation procedures and (4) spectral analysis in the time domain. The patients had significantly lower concentrations of phosphodiesters (PDE), 6.3±3.9 mM, and ATP-β, 3.6±1.1 mM, (P<0.05) compared with the control group (10.0±4.2 mM and 4.2±0.3 mM, respectively). The concentrations of phosphomonoesters (PME) were higher in the patient group, although this was not significant. Constructing an anabolic charge (AC) based on absolute concentrations, [PME]/([PME] + [PDE]), the patients had a significantly larger AC than the control subjects, 0.29 vs. 0.16 (P<0.005). Absolute concentration measurements of phosphorus metabolites in the liver are feasible using a slice selective sequence, and the technique demonstrates significant differences between patients and healthy subjects.

Needle stimulation (acupuncture) has recently been shown to increase blood flow in the tibialis anterior muscle and overlying skin in healthy subjects (HS) and patients with fibromyalgia (FM). The aim of the present study was to examine the effect of needle stimulation on local blood flow in the trapezius muscle and overlying skin in HS and two groups of patients suffering from chronic pain in the trapezius muscle, i.e., FM and work-related trapezius myalgia (TM) patients. Two modes of needling, deep muscle stimulation (Deep) and subcutaneous needle insertion (SC), were performed at the upper part of the shoulder and blood flow was monitored for 60 min post-stimulation. Blood flow changes were measured non-invasively by using a new application of photoplethysmography. Increased blood flow in the trapezius muscle and overlying skin was found in all three groups following both Deep and SC. In HS, Deep was superior to SC in increasing skin and muscle blood flow, whereas in FM, SC was as effective as, or even more effective, than Deep. In the severely affected TM patients, no differences were found between the stimuli, and generally, a lesser blood flow response to the stimuli was found. At Deep, the muscle blood flow increase was significantly larger in HS, compared to the two patient groups. Positive correlations were found between muscle blood flow at Deep and pressure pain threshold in the trapezius muscle, neck movement and pain experienced at the stimulation, and negative correlations were found with spontaneous pain-related variables, symptom duration and age, pointing to less favorable results with worsening of symptoms, and to the importance of nociceptor activation in blood flow increase. It was hypothesized that the different patterns of muscle blood flow response to the needling may mirror a state of increased sympathetic activity and a generalized hypersensitivity in the patients. The intensity of stimulation should be taken into consideration when applying local needle stimulation (acupuncture) in order to increase the trapezius muscle blood flow in chronic pain conditions.

Background. Dialysate-based, on-line measurements of Kt/V and protein catabolic rate (PCR) in dialysis patients have been considered more accurate compared with measurements on the blood side during dialysis. The primary aim of this study was to compare total removed urea (TRU) and PCR, normalized to body weight (nPCRw), obtained by three dialysate-based methods: (i) on-line ultraviolet (UV) absorbance of the spent dialysate; (ii) total dialysate collection (TDC), as reference method; and (iii) Urea Monitor 1000 (UM) from Baxter Healthcare Corp.

Methods. We studied 10 uraemic patients on chronic, thrice-weekly haemodialysis. We made absorption measurements (UV radiation) on-line with a spectrophotometer connected to the fluid outlet of the dialysis machine, with all spent dialysate passing through an optical cuvette for single-wavelength measurements. UV absorbance measurements were compared with TDC and the UM.

Results. nPCRw obtained with UV absorbance was 0.82±0.17, that from TDC 0.81±0.18, and that measured by UM 0.87±0.18, which was significantly higher than the results of the other methods. The difference between nPCRw calculated by TDC and by UM was –0.05±0.06, showing a slightly lower SD than the difference between nPCRw by TDC and UV absorbance, –0.01±0.07.

Conclusion. The study demonstrates that TRU, and consequently PCR, can be estimated by on-line measurement of the UV absorption in the spent dialysate.

The propagation of diagnostic ultrasonic imaging pulses in tissue and their interaction with contrast micro bubbles is a very complex physical process, which we assumed to be separable into three stages: pulse propagation in tissue, the interaction of the pulse with the contrast bubble, and the propagation of the scattered echo. The model driven approach is used to gain better knowledge of the complex processes involved. A simplified way of field simulation is chosen due to the complexity of the task and the necessity to estimate comparative contributions of each component of the process. Simulations are targeted at myocardial perfusion estimation. A modified method for spatial superposition of attenuated waves enables simulations of low intensity pulse pressure fields from weakly focused transducers in a nonlinear, attenuating, and liquid-like biological medium. These assumptions enable the use of quasi-linear calculations of the acoustic field. The simulations of acoustic bubble response are carried out with the Rayleigh-Plesset equation with the addition of radiation damping. Theoretical simulations with synthesised and experimentally sampled pulses show that the interaction of the excitation pulses with the contrast bubbles is the main cause of nonlinear scattering, and a 2-3 dB increase of second harmonic amplitude depends on nonlinear distortions of the incident pulse. The third heart sound is normally heard during auscultation of younger individuals but disappears with increasing age. However, this sound can appear in patients with heart failure and is thus of potential diagnostic use in these patients. Auscultation of the heart involves a high degree of subjectivity. Furthermore, the third heart sound has low amplitude and a low-frequency content compared with the first and second heart sounds, which makes it difficult for the human ear to detect this sound. It is our belief that it would be of great help to the physician to receive computer-based support through an intelligent stethoscope, to determine whether a third heart sound is present or not. A precise, accurate and low-cost instrument of this kind would potentially provide objective means for the detection of early heart failure, and could even be used in primary health care. In the first step, phonocardiograms from ten children, all known to have a third heart sound, were analysed, to provide knowledge about the sound features without interference from pathological sounds. Using this knowledge, a tailored wavelet analysis procedure was developed to identify the third heart sound automatically, a technique that was shown to be superior to Fourier transform techniques. In the second step, the method was applied to phonocardiograms from heart patients known to have heart failure. The features of the third heart sound in children and of that in patients were shown to be similar. This resulted in a method for the automatic detection of third heart sounds. The method was able to detect third heart sounds effectively (90%), with a low false detection rate (3.7%), which supports its clinical use. The detection rate was almost equal in both the children and patient groups. The method is therefore capable of detecting, not only distinct and clearly visible/audible third heart sounds found in children, but also third heart sounds in phonocardiograms from patients suffering from heart failure.

Blood and tissue velocity are measured and analysed in cardiac, vascular and other applications of diagnostic ultrasound (US). An error in system calibration is a potential risk for misinterpretation of the measurements. To determine the accuracy in velocity calibration, we tested three common commercial US systems using a Doppler string phantom. We tested pulsed and continuous-wave Doppler modes for velocities relevant to both cardiac blood flow and tissue-velocity estimation. The US systems were tested with settings and transducers commonly used in cardiac applications. One system consistently overestimated velocity by about 5%, whereas the other two systems were quite accurate in velocity estimation. These findings emphasize the importance of continuous quality control of US equipment.

We have investigated if features in the diffuse reflectance spectra from in vivo spectroscopic measurements of the tympanic membrane could aid the diagnosis of otitis media in children. Diffuse reflectance spectroscopy, in the visible wavelength range, was used in 15 ears from children with otitis media with effusion before and after myringotomy and in 15 healthy ears as a reference. Two previously published erythema detection algorithms yielded numerical quantities of haemoglobin content. With a combination of the algorithms, induced erythema (after myringotomy) was distinguished from healthy ears using Student's t-test (p < 0.01). Otitis media with mucous effusion was distinguished from (1) otitis media with serous effusion, (2) induced erythema and (3) healthy ears, (p < 0.05) using Student's t-test for independent groups and the paired t-test for dependent groups. Our results imply that reflectance spectroscopy is a promising technique to be used for the diagnosis of otitis media.

Acupuncture has become a widely used treatment modality in various musculoskeletal pain conditions. Acupuncture is also shown to enhance blood flow and recovery in surgical flaps. The mechanisms behind the effect on blood flow were suggested to rely on vasoactive substances, such as calcitonin gene-related peptide, released from nociceptors by the needle stimulation. In a previous study on healthy subjects, one needle stimulation into the anterior tibial muscle was shown to increase both skin and muscle blood flow. The aim of this study was to examine the effect of needle stimulation on local blood flow in the anterior tibial muscle and overlying skin in patients suffering from a widespread chronic pain condition. Fifteen patients with fibromyalgia (FM) participated in the study. Two modes of needling, deep muscle stimulation and subcutaneous needle insertion were performed at the upper anterior aspect of the tibia, i.e., in an area without focal pathology or ongoing pain in these patients. Blood flow changes were assessed non-invasively by photoplethysmography (PPG). The results of the present study were partly similar to those earlier found at a corresponding site in healthy female subjects, i.e., deep muscle stimulation resulted in larger increase in skin blood flow (mean (SE)): 62.4% (13.0) and muscle blood flow: 93.1% (18.6), compared to baseline, than did subcutaneous insertion (mean (SE) skin blood flow increase: 26.4% (6.2); muscle blood flow increase: 46.1% (10.2)). However, in FM patients subcutaneous needle insertion was followed by a significant increase in both skin and muscle blood flow, in contrast to findings in healthy subjects where no significant blood flow increase was found following the subcutaneous needling. The different results of subcutaneous needling between the groups (skin blood flow: p=0.008; muscle blood flow: p=0.027) may be related to a greater sensitivity to pain and other somatosensory input in FM.

One of the most frequently occuring artifacts in ultrasound imaging is reverberations. These are multiple reflection echoes that result in ghost echoes in the ultrasound image. A method for reducing these unwanted artifacts using a three-dimensional (3D) Wiener filter is presented. The Wiener filter is a global filter and produces an estimate of the uncorrupted signal by minimizing the mean square error between the estimate and the uncorrupted signal in a statistical sense. The procedure works as follows: In a graphic interface the operator is displayed an image sequence. The operator marks two areas in one of the images, one area which contains a typical reverberation artifact, and one area free from artifact. Using these areas to produce noise and signal estimates, a Wiener filter is created and applied to the sequence. The 3D Wiener filters display excellent selection capabilities, and the developed method significantly reduces the magnitude of the reverberation artifact in the tested sequences.

This paper describes a novel method for regional characterization of three-dimensional vector fields using a pattern matching approach. Given a three-dimensional vector field, the goal is to automatically locate, identify, and visualize a selected set of classes of structures or features. Rather than analytically defining the properties that must be fulfilled in a region in order to be classified as a specific structure, a set of idealized patterns for each structure type is constructed. Similarity to these patterns is then defined and calculated. Examples of structures of interest include vortices, swirling flow, diverging or converging flow, and parallel flow. Both medical and aerodynamic applications are presented in this paper.

Photoplethysmography (PPG) is a non-invasive optical way of measuring variations in blood volume and perfusion in the tissue, used in pulse oximetry for instance. Respiratory-induced intensity variations (RIIVs) in the PPG signal exist, but the physiological background is not fully understood. Respiration causes variations in the blood volume in the peripheral vascular bed. It was hypothesised that the filling of peripheral veins is one of the important factors involved. In 16 healthy subjects, the respiratory synchronous variations from a PPG reflection mode signal and the peripheral venous pressure (PVP) were recorded. Variations of tidal volume, respiratory rate and contribution from abdominal and thoracic muscles gave significant and similar amplitude changes in both RIIV and the respiratory variation of PVP (p < 0.01). The highest amplitudes of both signals were found at the largest tidal volume, lowest respiratory rate and during mainly thoracic breathing, respectively. The coherence between PVP and RIIV signals was high, the median (quartile range) being 0.78 (0.42). Phase analysis showed that RIIV was usually leading PVP, but variations between subjects were large. Although respiratory-induced variations in PVP and PPG showed a close correlation in amplitude variation, a causal relationship between the signals could not be demonstrated.

This study aims at investigating the radial dimensional changes, as a result of an applied intraluminal pressure for an elastic tube with non-uniform cross section. The study is related to a method for non-invasive pressure measurement using the extracorporeal tube as part of the sensor. The intended application is for monitoring of pressure in the blood and dialysate tubes during haemodialysis. The intention is to find a tube cross-section geometry that results in an expansion of the tube so that it is suitable to use as a component in a pressure sensor. The tube should have high radial compliance and expand in a well-defined manner to be able to transfer the intraluminal pressure to a transducer element sensing the radial force. Radial expansion was studied experimentally for tubes with different cross-section geometries. For small tube expansions the resolution in the experimental measurements was not sufficient to study the radial expansion. In this case, numerical simulation was performed. We conclude that a tube with essentially elliptic outer surface and circular inner surface, with a relation of 1:2 between the size of the thin and thick wall, results in a radial expansion upon application of pressure indicating that this tube is suitable for use as part of a sensor.

In 14 healthy female subjects, the effects of needle stimulation (acupuncture) on skin and muscle blood flow were investigated using a non-invasive custom-designed probe and photoplethysmography (PPG). In randomised order, 2–7 days apart, three modes of needle stimulation were performed on the anterior aspect of the tibia: superficial insertion (SF), insertion into the anterior tibial muscle (Mu), and insertion into the muscle including manipulation of the needle in order to elicit a distinct sensation of distension, heaviness or numbness (DeQi). Before intervention, the subjects rested for 30 min. After the intervention, the needle was left in situ for 20 min. Blood flow recordings were performed intermittently from 10 min prior to the intervention to the end of the trial. In a fourth session, serving as control, corresponding measurements were performed without any needle stimulation. Area under curve was calculated for 5-min periods prior to and after stimulation, respectively, and for the remaining 15-min period after stimulation. Compared to the control situation, muscle blood flow increased following both Mu and DeQi for 20 min, with the latter being more pronounced for the initial 5 min. Skin blood flow increased for 5 min following DeQi. However, no increase was found following SF. The DeQi stimulation was preceded by higher visual analogue scale ratings of anxiety prior to stimulation, which might have influenced skin blood flow to some extent. The results indicate that the intensity of the needling is of importance, the DeQi stimulation resulting in the most pronounced increase in both skin and muscle blood flow.

Purpose: The aim of this work was to describe a new optical method for monitoring solutes in a spent dialysate using absorption of UV radiation. Method: The method utilises UV-absorbance determined in the spent dialysate using a spectrophotometrical set-up. Measurements were performed both on collected dialysate samples and on-line. During on-line monitoring, a spectrophotometer was connected to the fluid outlet of the dialysis machine, with all spent dialysate passing through a specially-designed cuvette for optical single-wavelength measurements. The concentrations of several substances of various molecular sizes, electrical charge, transport mechanism, etc. were determined in the dialysate and in the blood using standard laboratory techniques. The correlation coefficient between UV-absorbance of the spent dialysate and concentration of the substances in the spent dialysate and in the blood was calculated from data based on the collected samples. Results: The obtained on-line UV-absorbance curve demonstrates the possibility to follow a single hemodialysis session continuously and to monitor deviations in the dialysator performance using UV-absorbance. The experimental results indicate a very good correlation between UV-absorbance and several small waste solutes removed such as urea, creatinine and uric acid in the spent dialysate and in the blood for every individual treatment at a fixed wavelength of 285 nm. Moreover, a good correlation between the UV-absorbance and substances like potassium, phosphate and 2-microglobulin was obtained. The lowest correlation was achieved for sodium, calcium, glucose, vitamin B12 and albumin. Conclusions: A technique for on-line monitoring of solutes in the spent dialysate utilising the UV- absorbance was developed. On-line monitoring during a single hemodialysis session exploiting UV- absorbance represents a possibility to follow a single hemodialysis session continuously and monitor deviations in dialysis efficiency (e.g. changes in blood flow and clearance). The UV-absorbance correlates well to the concentration of several solutes known to accumulate in dialysis patients indicating that the technique can be used to estimate the removal of retained substances.

A four-dimensional mapping (three spatial dimensions + time) of myocardial strain-rate would help to describe the mechanical properties of the myocardium, which affect important physiological factors such as the pumping performance of the ventricles. Strain-rate represents the local instantaneous deformation of the myocardium and can be calculated from the spatial gradients of the velocity field. Strain-rate has previously been calculated using one-dimensional (ultrasound) or two-dimensional (2-D) magnetic resonance imaging) techniques. However, this assumes that myocardial motion only occurs in one direction or in one plane, respectively. This paper presents a method for calculation of the time-resolved three-dimensional (3-D) strain-rate tensor using velocity vector information in a 3-D spatial grid during the whole cardiac cycle. The strain-rate tensor provides full information of both magnitude and direction of the instantaneous deformation of the myocardium. A method for visualization of the full 3-D tensor is also suggested. The tensors are visualized using ellipsoids, which display the principal directions of strain-rate and the ratio between strain-rate magnitude in each direction. The presented method reveals the principal strain-rate directions without a priori knowledge of myocardial motion directions.

Even short time delays (less than 30 ms) in cardiac motion pattern may have clinical relevance. These delays can be measured with echocardiography, using techniques such as flow and tissue Doppler and M-mode together with external signals (e.g., ECG and phonocardiography). If one or more of these signals are delayed in relation to the other signals (asynchronous), an incorrect definition of cardiac time intervals can occur, the consequence of which is invalid measurement. To determine if this time delay in signal processing is a problem, we tested three common ultrasound (US) systems using the ECG as the reference signal. We used a digital ECG simulator and a Doppler string phantom to obtain test signals for flow and tissue pulsed Doppler, M-mode, phonocardiography, auxiliary and ECG signals. We found long time delays of up to 90 ms in one system, whereas delays were mostly short in the two other systems. The time delays varied relative to system settings. Consequently, to avoid these errors, precise knowledge of the characteristics of the system being used is essential.

Spectacular color changes of fishes, frogs and other lower vertebrates are due to the motile activities of specialized pigment containing cells. Pigment cells are interesting for biosensing purposes since they provide an easily monitored physiological phenomenon. Melanophores, containing dark brown melanin pigment granules, constitute an important class of chromatophores. Their melanin-filled pigment granules may be stimulated to undergo rapid dispersion throughout the melanophores (cells appear dark), or aggregation to the center of the melanophores (cells appear light). This simple physiological response can easily be measured in a photometer. Selected G protein coupled receptors can be functionally expressed in cultured frog melanophores. Here, we demonstrate the use of recombinant frog melanophores as a biosensor for the detection of opioids. Melanophores were transfected with the human opioid receptor 3 and used for opiate detection. The response to the opioid receptor agonist morphine and a synthetic opioid peptide was analyzed by absorbance readings in an aggregation assay. It was shown that both agonists caused aggregation of pigment granules in the melanophores, and the cells appeared lighter. The pharmacology of the expressed receptors was very similar to its mammalian counterpart, as evidenced by competitive inhibition by increasing concentrations of the opioid receptor inhibitor naloxone. Transfection of melanophores with selected receptors enables the creation of numerous melanophore biosensors, which respond selectively to certain substances. The melanophore biosensor has potential use for measurement of substances in body fluids such as saliva, blood plasma and urine.

The effects of limb elevation and increased intramuscular pressure (IMP) on blood flow in the tibialis anterior muscle and leg neuromuscular function were studied in eight healthy subjects. Muscle blood flow (MBF) was measured by photoplethysmography using a custom-designed probe. IMP was elevated bilaterally by vein obstruction (60-65 mmHg) lasting 30 min induced by a thigh tourniquet of casted legs. Skin sensibility of the feet and the amplitude and area of the compound muscle action potentials from the extensor digitorum brevis muscle were evaluated. The subject kept one leg elevated 32 cm above heart level and the contralateral leg at heart level. All recordings were made before, during and after vein obstruction. IMP increased to 40 mmHg in the vein-obstructed casted legs. Perfusion pressure decreased from [mean (SD)] 42 (5.8) mmHg to 17 (6.4) mmHg in the elevated leg and from 65 (9.9) mmHg to 43 (8.4) mmHg in the non-elevated leg. MBF decreased by 50% in the elevated leg and by 42% in the non-elevated leg. Subjects experienced sensory dysfunction and muscular weakness in the elevated leg. In conclusion, increased IMP, induced by venous obstruction of a casted leg, reduced perfusion pressure and MBF, and resulted in a diminished amplitude and area of the compound muscle action potentials. Limb elevation above heart level combined with venous stasis of a casted leg further reduced perfusion pressure and MBF, and induced sensory dysfunction and muscular weakness.

We used photoplethysmography (PPG) to monitor blood flow changes in the human anterior tibial muscle during arterial occlusion and during isometric and concentric contractions. Single-fibre laser-Doppler flowmetry (LDF) was used as a reference in 12 healthy subjects (5 men, 7 women, mean age 24 years). Post-exercise hyperaemic muscle blood flow (MBF) was measured immediately after isometric dorsiflexion of the ankle joint at maximal contraction for 1 min and full range-of-motion dorsiflexion and plantar flexion of the ankle joint for 1 min. A thigh tourniquet was applied for the evaluation of post-occlusive reactive hyperaemia. The MBF (baseline=100%) was [mean (SD)] 150 (31)% (P = 0.003) by PPG (880 nm) and 182 (66)% (P = 0.012) by LDF. After 1 min of maximal isometric contraction, MBF increased to 150 (51)% (P = 0.003) by PPG (880 nm) and to 169 (43)% (P = 0.005) by LDF. After 1 min of maximal concentric contractions, MBF increased to 158 (59)% (P = 0.003) by PPG (880 nm) and to 170 (99)% (P = 0.008) by LDF. Skin blood flow, PPG (560 nm), did not change significantly after isometric or concentric contractions. The results indicate that reactive hyperaemia after exercise and arterial occlusion can be assessed in the human anterior tibial muscle using PPG.

Black pigment cells, melanophores, e.g. located in the epidermis and dermis of frogs, are large flat cells having intracellular black pigment granules, called melanosomes. Due to a large size, high optical contrast, and quick response to drugs, melanophores are attractive as biosensors as well as for model studies of intracellular processes; e.g. organelle transport and G-protein coupled receptors. The geometry of melanosomes from African clawed toad, Xenopus laevis, has been measured using scanning force microscopy (SFM). Three-dimensional images from SFM were used to measure height, width, and length of the melanosomes (100 from aggregated cells and 100 from dispersed cells). The volumes of melanosomes isolated from aggregated and dispersed melanophores were significantly different (P<0.05, n=200). The average ellipsoidal volume was 0.14±0.01 (aggregated) and 0.17±0.01 mum3 (dispersed), a difference of 18%. The average major diameter was 810±20 and 880±20 nm for aggregated and dispersed melanosomes, respectively. To our knowledge, this is the first time SFM has been used to study melanosomes. This may provide an alternative non-destructive technique that may be particularly suitable for studying morphological aspects of various melanin granules.

Objectives: We sought to evaluate the Doppler assessment of effective orifice area in aortic prosthetic valves. The effective orifice area is a less flow-dependent parameter than Doppler gradients that is used to assess prosthetic valve function. However, in vivo reference values show a pronounced spread of effective orifice area and smaller orifices than expected compared with the geometric area. Methods: Using Doppler echocardiography, we studied patients who received a bileaflet St Jude Medical valve (n = 75, St Jude Medical, Inc, St Paul, Minn) or a tilting disc Omnicarbon valve (n = 46, Medical CV, Incorporated, Inver Grove Heights, Minn). The prosthetic valves were also investigated in vitro in a steady flow model with Doppler and catheter measurements in the different orifices. The effective orifice area was calculated according to the continuity equation. Results: In vivo, there was a wide distribution with the coefficient of variation (SD/mean ╫ 100%) for different valve sizes ranging from 21% to 39% in the St Jude Medical valve and from 25% to 33% in the Omnicarbon valve. The differences between geometric orifice area and effective orifice area in vitro were 1.26 ▒ 0.41 cm2 for St Jude Medical and 1.17 ▒ 0.38 cm2 for Omnicarbon valves. The overall effective orifice areas and peak catheter gradients were similar: 1.35 ▒ 0.37 cm2 and 25.9 ▒ 16.1 mm Hg for St Jude Medical and 1.46 ▒ 0.49 cm2 and 24.6 ▒ 17.7 mm Hg for Omnicarbon. However, in St Jude Medical valves, more pressure was recovered downstream, 11.6 ▒ 6.3 mm Hg versus 3.4 ▒ 1.6 mm Hg in Omnicarbon valves (P = .0001). Conclusions: In the patients, we found a pronounced spread of effective orifice areas, which can be explained by measurement errors or true biologic variations. The in vitro effective orifice area was small compared with the geometric orifice area, and we suspect that nonuniformity in the spatial velocity profile causes underestimation. The St Jude Medical and Omnicarbon valves showed similar peak catheter gradients and effective orifice areas in vitro, but more pressure was recovered in the St Jude Medical valve. The effective orifice area can therefore be misleading in the assessment of prosthetic valve performance when bileaflet and tilting disc valves are compared.

OBJECTIVE: To test the accuracy and reproducibility of systemic cardiac output (CO) measurements using surface integration of velocity vectors (SIVV) in a pediatric animal model with hemodynamic instability and to compare SIVV with traditional pulsed-wave Doppler measurements.

DESIGN: Prospective, comparative study.

SETTING: Animal research laboratory at a university medical center.

SUBJECTS: Eight piglets weighing 10-15 kg.

INTERVENTIONS: Hemodynamic instability was induced by using inhalation of isoflurane and infusions of colloid and dobutamine.

MEASUREMENTS: SIVV CO was measured at the left ventricular outflow tract, the aortic valve, and ascending aorta. Transit time CO was used as the reference standard.

RESULTS: There was good agreement between SIVV and transit time CO. At high frame rates, the mean difference +/- 2 SD between the two methods was 0.01+/-0.27 L/min for measurements at the left ventricular outflow tract, 0.08+/-0.26 L/min for the ascending aorta, and 0.06+/-0.25 L/min for the aortic valve. At low frame rates, measurements were 0.06+/-0.25, 0.19+/-0.32, and 0.14+/-0.30 L/min for the left ventricular outflow tract, ascending aorta, and aortic valve, respectively. There were no differences between the three sites at high frame rates. Agreement between pulsed-wave Doppler and transit time CO was poorer, with a mean difference +/- 2 SD of 0.09+/-0.93 L/min. Repeated SIVV measurements taken at a period of relative hemodynamic stability differed by a mean difference +/-2 SD of 0.01+/-0.22 L/min, with a coefficient of variation = 7.6%. Intraobserver coefficients of variation were 5.7%, 4.9%, and 4.1% at the left ventricular outflow tract, ascending aorta, and aortic valve, respectively. Interobserver variability was also small, with a coefficient of variation = 8.5%.

CONCLUSIONS: SIVV is an accurate and reproducible flow measurement technique. It is a considerable improvement over currently used methods and is applicable to pediatric critical care.

The extent to which Doppler echocardiography information can be used in the assessment of prosthesis hemodynamic performance is still controversial. The goals of our study were to assess the importance of valve design and size both on Doppler echocardiography findings and on pressure recovery in a fluid mechanics model. We performed Doppler and catheter measurements in the different orifices of the bileaflet St Jude (central and side orifices), the monoleaflet Omnicarbon (major and minor orifices), and the stented Biocor porcine prosthesis. Net pressure gradients were predicted from Doppler flow velocities, assuming either independence or dependence of valve size. The peak Doppler estimated gradients (mean +/- SD for sizes 21 to 27) were 21 +/- 10.3 rum Hg for St Jude, 18 +/- 9.3 mm Hg for Omnicarbon, and 37 +/- 14.5 mm Hg for Biocor (P <.05 for St Jude and Omnicarbon vs Biocor). The pressure recovery (proportion of peak catheter pressure) was 53% +/- 8.6% for central-St Jude, 29% +/- 8.9% for side-St Jude, 20% +/- 5.6% for major-Omnicarbon, 23% +/- 7.4% for minor-Omnicarbon, and 18% +/- 3.6% for Biocor (P <.05 for central-St Jude and side-St Jude vs Omnicarbon and Biocor). Valve sizes (2) significantly influenced pressure recovery (y in percentage) (central-St Jude: y = 3.7x - 35.9, r = 0.88, P =.0001, major-Omnicarbon: y = 2.1x - 30.3, r = 0.85, P =.0001). By assuming dependence of valve size, Doppler was able to predict net pressure gradients in St Jude with a mean difference between net catheter and Doppler-predicted gradient of - 3.8 +/- 2.5 mm Hg. In conclusion, prosthetic value design and size influence the degree of pressure recovery, making Doppler gradients potentially misleading in both the assessment of hemodynamic performance and the comparison of one design with another. The preliminary results indicate that net gradient can be predicted from Doppler gradients,

The purpose of this paper is to explain theoretically the origin of previously presented experimental results by an optical non-invasive method using NIR for imaging blood vessels based on a specific combination of several physical parameters. The theoretical model is based on the diffusion approximation derived from the transport theory deep in a bulk tissue. An analytical solution was obtained describing photon behaviour under certain conditions during vessel identification. The modelled results indicate that the vessel identification facility depends upon source-detector separation and vessel depth, and does not depend essentially on the radiant power from the light source. The solution offers a relatively simple theoretical explanation of the experimental results and can be applied to several other clinical applications using similar technical solutions.

Repeatability of the method was tested at various flow rates for each angle of insonation and vessel. In a univentricular pulsatile model, SIVV flow measured at the mitral inlet was compared to true flow (29-61 mL/s). Correlation was excellent for the 19-mm vessel (r2= 0.99). There was a systematic bias but close limits of agreement (mean ▒ 2 SD = -24.1% ▒ 7.6% at 45 ░, +16.4% ▒ 11.0% at 60 ░). Using the 11-mm vessel, a quadratic relationship was demonstrated between between SIVV and true flow (r2 = 0.98- 0.99), regardless of the angle of insonation. In the pulsatile system, good agreement and correlation were shown (r2 = 0.94, mean ▒ 2 SD = -4.7 ▒ 10.1%). The coefficients of variation for repeated SIVV measurements ranged from 0.9% to 10.3%. This method demonstrates precision and repeatability, and is potentially useful for clinical measurements. The acceleration-induced displacement artifact impairs the accuracy of MR velocity measurements. This study proposes a post processing method for correction of this artifact. Velocity measurements were performed in a flow phantom containing a constriction. Velocity curves were obtained from streamlines parallel to the frequency, phase, and slice directions, respectively. The acceleration-induced displacement artifact was most prominent when the frequency encoding direction was aligned with the flow direction. After correction, velocity assignment improved and a more accurate description of the flow was obtained. In vivo measurements were performed in the aorta in a patient with a repaired aortic coarctation. The correction method was applied to velocity data along a streamline parallel to the frequency encoding direction. Objective. Photoplethysmography (PPG) is a non-invasive optical technique that measures variations in skin blood volume and perfusion. The PPG signal contains components that are synchronous with respiratory and cardiac rhythms. We undertook this study to evaluate PPG for monitoring patients' respiratory rate in the postoperative care unit, using a new prototype device. We compared it with the established technique, transthoracic impedance (TTI). Methods. PPG signals from 16 patients (ASA classes 1-2, mean age 43 years) who were recovering from general anaesthesia after routine operations were recorded continuously for 60 minutes/patient. The respiratory synchronous part of the PPG signal was extracted by using a bandpass filter. Detection of breaths in the filtered PPG signals was done both visually and by using an automated algorithm. In both procedures, the detected breaths were compared with the breaths detected in the TTI reference. Results. A total of 10.661 breaths were recorded, and the mean+/-SD respiratory rate was 12.3 +/- 3.5 breaths/minute. When compared with TTI, the rates of false positive and false negative breaths detected by PPG (visual procedure) were 4.6+/-4.5% and 5.8 +/- 6.5%, respectively. When using the algorithm for breath detection from PPG, the rates of false positive and false negative breaths were 11.1 +/- 9.7% and 3.7+/- 3.8%, respectively, when compared to TTI. Lower respiratory rates increased the occurrence of false-positive breaths that were detected by the PPG using visual identification (p < 0.05). The same tendency was seen with the automated PPG procedure (p < 0.10). Conclusions. Our results indicate that PPG has the potential to be useful for monitoring respiratory rate in the postoperative period.

It is well known that the flow of air through the trachea during respiration causes vibrations in the tissue near the trachea, which propagate to the surface of the body and can be picked up by a microphone placed on the throat over the trachea. Since the vibrations are a direct result of the airflow, accurate timing of inspiration and expiration is possible. This paper presents a signal analysis solution for automated monitoring of breathing and calculation of the breathing frequency. The signal analysis approach uses tracheal sound variables in the time and frequency domains, as well as the characteristics of the disturbances that can be used to discriminate tracheal sound from noise. One problem associated with the bioacoustic method is its sensitivity for acoustic disturbances, because the microphone tends to pick up all vibrations, independent of their origin. A signal processing method was developed that makes the bioacoustic method clinically useful in a broad variety of situations, for example in intensive care and during certain heart examinations, where information about both the precise timing and the phases of breathing is crucial.

Quantitative computerized analysis of data from myocardial thallium-201 (201Tl) single-photon emission tomography (SPET) may improve the diagnostic accuracy of coronary heart disease. The reference ranges for post-menopausal women are, however, limited and obtained mainly from patients. To compare reference values from healthy post-menopausal women and to improve the quantitative analysis, 20 women (10 patients with coronary heart disease and previous infarction and 10 age-matched healthy volunteers) were examined immediately post exercise and after a delay. A nine-segment 'bull's-eye' model was used for analysis. At visual evaluation, reproducibility was high (93%), no false-positive results were obtained and in 70% of the patients the SPET was interpreted as abnormal. Using reported reference values for quantitative analysis, all the healthy women had an abnormal result. New reference values based on three different methods of 'normalization' were calculated: the relative activity of segment 3 set to 100%, the segment with the highest activity set to 100% and a least-squares method. They all differed significantly from those that had previously been reported. The frequencies of agreement between visual and quantitative analysis were 84-92% and were highest when segment 3 was used as a reference, but in this case only 40% of the patients with coronary heart disease had an abnormal SPET. Using the least-squares method for handling digital information, the SD of the normal values decreased and 90% of the patients with coronary heart disease were accurately diagnosed. These results provide quantitative digital reference values for healthy post-menopausal women. They verify that quantitative analysis is in diagnostic agreement with visual evaluation, stress the need for local verification of reference ranges and suggest a least-square normalization method for the analysis.

Two moving string test targets have been developed and used to characterize ultrasound Doppler instrumentation. The characteristics investigated were sample volume size and location. The tests were performed on a combined echo/Doppler instrument and on an annular array system. The procedures can be carried out routinely in the clinical laboratory to ensure that the instrument is working properly or as an aid for correct interpretation of acquired data.

In a clinical study of oesophageal manometry with fluid-filled catheters, both a non-perfused system and a perfused system with a syringe pump have been compared to a system with a low-compliance perfusion pump, which served as a reference. Significantly lower values of motility amplitudes, motility derivatives, and partly of LES pressures, and a time delay of up to 0.5 sec of the amplitude maximum were obtained with the non-perfused system and the system with a syringe pump in comparison to the low-compliance system. Since the oesophageal function can be erroneously evaluated by use of a non-perfused system or a perfused system with a syringe pump, such systems cannot be recommended for clinical use.

A device for measuring the difference in level between the pressure transducer and a point of measurment is described. It can be used in oesophageal manometry with waterfilled catheters to measure and compensate for superimposed hydrostatic pressure. The practical application of the method is illustrated.

The aim of this work was to describe a new method for optical monitoring of solutes in a spent dialysate. The method utilizes UV light absorption employing a commercially available spectrophotometer. Measurements were performed both on collected dialysate samples and on-line. The concentration of several removed solutes and electrolytes in the serum and in the dialysate was determined simultaneously using standard laboratory techniques. During on-line monitoring the spectrophotometer was connected to the fluid outlet of the dialysis machine. On-line measurements during a single hemodialysis session demonstrated a possibility to monitor deviations in the dialysator performance (e.g. dialysator in bypass). The experimental results indicated a good correlation between UV absorption and several removed solutes (urea, creatinine) in the spent dialysate. The correlation coefficient for urea and creatinine concentrations in the dialysate was very high for every individual treatment. The UV absorbance correlates well to the concentrations of several solutes thought to be uremic toxins. The results indicate that the technique can be used as a continuous, on-line method for monitoring deviations in the dialysator performance and may estimate the removal of the overall toxins. In the future, the new method will be used to evaluate parameters describing delivery of the prescribed treatment dose such as KT/V and Urea Reduction Rate (URR).

Blood and tissue velocities are measured and analyzed in cardiac, vascular, and other applications of diagnostic ultrasound. Errors in system performance might give invalid measurements.

We developed two moving string test targets and a rotating cylinder phantom (Doppler phantoms) to characterize Doppler ultrasound systems. These phantoms were initially used to measure such variables as sample volume dimensions, location of the sample volume, and the performance of the spectral analysis. Later, specific tests were designed and performed to detect errors in signal processing, causing time delays and inaccurate velocity estimation in all Doppler modes.

In cardiac motion pattern even time delays as short as 30 ms may have clinical relevance. These delays can be obtained with echocardiography by using flow and tissue Doppler and M-mode techniques together with external signals (e.g., electrocardiography (ECG) and phonocardiography). If one or more of these signals are asynchronous in relation to the other signals, an incorrect definition of cardiac time intervals may occur. To determine if such time delays in signal processing are a serious problem, we tested four commercial ultrasound systems. We used the Doppler string phantom and the rotating cylinder phantom to obtain test signals. We found time delays of up to 90 ms in one system, whereas delays were mostly short in the other systems. Further, the time delays varied relative to system settings. In two-dimensional (2D) Doppler the delays were closely related to frame rate.

To determine the accuracy in velocity calibration, we tested the same four ultrasound systems using the Doppler phantoms to obtain test signals for flow (PW) and tissue (T-PW) pulse Doppler and for continuous wave (CW) Doppler. The ultrasound systems were tested with settings and transducers commonly used in cardiac applications. In two systems, the observed errors were mostly close to zero, whereas one system systematically overestimated velocity by an average of 4.6%. The detected errors are mostly negliable in clinical practice but might be significant in certain cases and research applications.

Phonocardiographic signals contain bioacoustic information reflecting the operation of the heart. Normally there are two heart sounds, and additional sounds indicate disease. If a third heart sound is present it could be a sign of heart failure whereas a murmur indicates defective valves or an orifice in the septal wall. The primary aim of this thesis is to use signal processing tools to improve the diagnostic value of this information. More specifically, three different methods have been developed:

• A nonlinear change detection method has been applied to automatically detect heart sounds. The first and the second heart sounds can be found using recurrence times of the first kind while the third heart sound can be found using recurrence times of the second kind. Most third heart sound occurrences were detected (98 %), but the amount of false extra detections was rather high (7 % of the heart cycles).

• Heart sounds obscure the interpretation of lung sounds. A new method based on nonlinear prediction has been developed to remove this undesired disturbance. High similarity was obtained when comparing actual lung sounds with lung sounds after removal of heart sounds.

• Analysis methods such as Shannon energy, wavelets and recurrence quantification analysis were used to extract information from the phonocardiographic signal. The most prominent features, determined by a feature selection method, were used to create a new feature set for heart murmur classification. The classification result was 86 % when separating patients with aortic stenosis, mitral insufficiency and physiological murmurs.

The derived methods give reasonable results, and they all provide a step forward in the quest for an intelligent stethoscope, a universal phonocardiography tool able to enhance auscultation by improving sound quality, emphasizing abnormal events in the heart cycle and distinguishing different heart murmurs.
